When calling to schedule a new patient appointment, please set aside approximately 10 minutes for the intake process when calling. Please have all of the information below available.
- We will need all demographics to include full name, date of birth, address, cell phone number, and an email address is required.
- We will need a list of all current medication to include dosage and instructions for each medication.
- We will need to know what pharmacy you want to have prescriptions sent to and the address of the location of your choice (phone number if available)
- Once scheduled you will complete all consents and intake forms through our patient portal that you will receive information on
**ALL PATIENTS: It is patient responsibility to update all contact and demographic information upon any changes by calling our office and either speaking with someone or leaving a detailed voicemail with the information that needs to be updated.**
If you are a RETURN patient: Please only use this method if you are an ESTABLISHED patient. Thank you.
Established patients can request appointments through their patient portal (this is the preferred method) CLICK HERE to go to the portal.
Once logged in, click 'appointment' tab then click 'Book appointment'. Please be sure to choose Stephanie Johnson, NP. Or you can call the office to schedule a return appointment.
